Gas under the diaphragm and general peritonitis, but no perforation found at operation

A 30-year-old man (addicted to many drugs) underwent laparotomy for signs of perititonitis and free gas under the diaphragm on X-rays and CT; however, although the abdominal cavity was full of pus and flakes which was thoroughly washed clean, no GI perforation could be found despite repeated thorough checks. Closed with drain; rapidly recovered and discharged 2 days later.
