Incidentally, the Rouviere’s sulcus in this patient was jus a slit.

An old man (65 years old) reported to have a mass in the caecum (on CT scan) turned out, at operation, to have an appendix abscess with gangrene  of the wall of the caecum, for which resection of the caecum and an end-to-end leocolic anastomosis had to be done.

A difficult lap chole in an obese 55 years old hypertensive lady (dr SK Jindal’s relative). The gallbladder was almost entirely intrahepatic, thick-walled and packed with large stones. the stones were removed first to empty the gallbladder and helping to grasp the thick walls of the gallbladder. Since the Calot’s triangle was obliterated, the gallbladder neck was ligated with vicryl, and the gallbladder removed in pieces since it would not otherwise come out through the 10 mm port. The patient remained in ICU for one day, but recovered very well.

Attended the free medical camp at Baru Sahib. Performed 12 open cholecystectomies (laparoscopic equipment not available), 2 PUH repairs, one hydrocelectomy, one cystolithotomy, and 2 inguinal hernia repairs, besides excision of a couple of lipomas.

Assisted Dr Sukha Singh at TAH at his clinic (Amrit hospital) in Amritsar. Elderly lady with atrophic uterus. ? indicated or not?. Postop had severe abdominal pain and distension and vomiting, frightening the surgical team but eventually settled in 3 days.

Also to remember was the first laparoscopic nephrectomy for non-functioning hydronephrosis (R). Very satisfactory. Specimen removed in a bag. Enjoyed.

two difficult cases:
a huge man 25 years old but very obese , more than 150 kg. Varicose veins ligated at saphenofemoral junction (Trendelenburg) plus multiple phlebecotomies.
Anderson Hynes pyeloplasty in a 7 years old male child. Stent not available. Drained.

Day case lap chole number 6: for the first time, port site (epigastric) bleeding caused a nuisance, controlled afterwards (after removing the gallbladder) by two sutures with port-closure needle.

A disaster: trochar vascular injury during first trochar insertion for the camera in a case of diagnostic laparoscopy at Sehat, injuring the aortofemoral junction on the right side, leading to shock and a big haematoma, called in to repair, but unsuccessful. Patient succumbed. Sad day.

Emergency call for caesarean hysterectomy. Patient in shock. Saved after a quick hysterectomy at Babbar’s.

ICS conference:
Went to Delhi in the evening with 3 others. Then from Delhi to Chennai by air, Chennai to Trichi by train, reaching Trichi by 11 pm.
16,17,18: Stayed at Trichi for the annual conference of ICS (IS).
18th evening: from Trichi to Chennai by train.
19.10.08: Chennai to Delhi by air, and Delhi to Patiala by taxi, reaching back home by 7 pm.

At a lap appy,  an area over ileum was inadvertently scored with diathermy , closed with 3-0 silk.
