An obstructed paraumbilical hernia (dilated bowel loops with air-fluid levels on X-ray), was explored under spinal anesthesia in a 45 years old male, addicted to opium. The obstructed small bowel loop was found to be strangulated with changes of gangrene in it. The strangulated loop was released, the gangrenous part was resected and an end-to-end anastomosis performed. The hernial defect was then repaired without a prolene mesh.
